Sometimes it’s just unlucky. The cone (dome, ear tip etc) is a perfect fit. It makes a seal, a vacuum when you remove it, and this makes it pop off the end of the receiver. This is usually when the dome has no ventilation holes.
With domes that do have vent holes in, if they keep coming off, it’s likely that they are either:
Too loose fitting on the receiver to start with
Too loose from being taken on/off too many times
They have got greasy somehow. If you use lotion, moisturisers or cleaning wipes that leave residue, that may be the reason.
